Fax the referral slip and current radiographs to (512) 346-8135, or call (512) 346-6097 and ask for our treatment plan coordinator. For scans and images, we send your office a secure upload link the same day. Please don't email patient images or forms.
A secure online referral with image upload is being set up and will appear here when it is live. It will ask for:
- Your office, doctor, and how you want the report sent
- Patient name, date of birth, phone, and insurance
- Reason for referral and tooth numbers, with urgency
- Medical alerts, sedation request, and the restorative plan for implant cases
- Radiographs, photos, and periodontal charting as uploads

What Happens Next
- We confirm receipt to your office the same business day and send you the consult date once it’s booked.
- We call and text your patient the same business day. Three attempts over a week; if we can’t reach them, we tell you.
- Consult within five business days, or same day for emergencies during office hours (Monday through Thursday; referrals that arrive after 3 PM or on a Friday are called the next business morning by 9 AM).
- A written report to your office: diagnosis, plan, radiographs, the maintenance interval, and for implants the fixture details your lab needs.
- Your patient returns to you for restorative care and alternating maintenance.
Emergencies
Call and say it’s urgent. We will see the patient today during office hours. A gum abscess, a suddenly loose tooth, or swelling around an implant qualifies.
Questions About a Case
Ask for Dr. Verrett when you call. If he’s with a patient, he calls back the same business day.
