Once your consult is scheduled we send you a link to complete your medical history securely before the visit. Completing it ahead saves about fifteen minutes at the front desk.
New Patient Forms
- Medical history and registration. Sent to you by secure link when your consult is scheduled. Prefer paper? Call (512) 346-6097 and we’ll mail it, or arrive ten minutes early and complete it here.
- Insurance information. Bring your card; we copy it at the visit.
If You Are Having Sedation
- Sedation companion information. Given to you at the consult; your driver reads and signs it.
- Pre-appointment instructions. On the before and after your procedure page.
Other Forms
- Request for release of records. To send your records to another office, or to have another office send records to us. Call and we’ll send it, or sign it at the front desk.
- Medical consultation form. Used when we need clearance from your physician before treatment. We send this to your physician directly.
A Note on Privacy
Please don’t send completed forms or health information by ordinary email. Bring the printed form, complete the secure link, or hand it to us at the desk. Our Notice of Privacy Practices explains how your information is protected.
